Widow IRMAA: why the income line drops from $218,000 to $109,000

Updated August 2026 · Educational, not tax advice

Married filing jointly uses a $218,000 first IRMAA line (2024 MAGI → 2026 premium). Single filers and qualifying surviving spouses use $109,000 for the same tier. The same dollar amount of income can be under the line as a couple and over as a survivor — or the reverse after a death changes filing status. This article flags the issue; it is not a full survivor tax or estate plan.

Two columns, same MAGI

Example: $180,000 MAGI.

The $99/year household board includes a widow view so you can see the same MAGI against both columns. The free check can still be run for the household you have today (for example, married filing jointly).

Qualifying surviving spouse (QSS)

QSS uses the individual IRMAA table ($109,000 first line), not the joint $218,000 line — same as single and head of household for IRMAA purposes (CMS, Nov 14, 2025). Tax filing status and Medicare premium tables do not always move in lockstep; confirm both with a CPA.

After a death: IRMAA and SSA-44

Income may drop after a spouse dies. If SSA is still using a high lookback year, Form SSA-44 may let SSA use a more recent return. Official door: ssa.gov/medicare/lower-irmaa.

Keep this household on the $99/year board. Lookback + this-year clocks, five-year path, widow view, CPA print, CMS updates.
